This comprehensive data integration allows the tool to accurately identify the prevalence of problematic prescribing habits that lead to polypharmacy. \r\n\r\nThe system is designed to integrate seamlessly with existing electronic clinical decision support platforms. Once integrated, it provides physicians, nurse practitioners, and pharmacists with actionable insights to identify high-impact deprescribing opportunities.
