EVOLV-Rx functions by utilizing a curated set of the 18 most scientifically valid and clinically useful LVP practices, which were established via an expert Delphi Panel alongside patient and caregiver input. The software applies sophisticated algorithms across diverse administrative healthcare data, including ICD-10 diagnosis codes, CPT codes, National Drug Codes (NDCs), and patient sociodemographic variables. This comprehensive data integration allows the tool to accurately identify the prevalence of problematic prescribing habits that lead to polypharmacy. The system is designed to integrate seamlessly with existing electronic clinical decision support platforms. Once integrated, it provides physicians, nurse practitioners, and pharmacists with actionable insights to identify high-impact deprescribing opportunities. By focusing on medications where discontinuation is most likely to be acceptable to patients, the technology bridges the gap between clinical data and person-centered care.
